Custom T-shirts

I was wondering what people thought the best place to get limited run custom t-shirts made.  I know cafepress.com does this, but all the hoops the make you jump through is a little annoying.

If you're looking for just a limited run, you'll find screen printing shops close to home.  It's cutthroat competition, so you can probably check around to get the best price.
