I just saw a mention of:

Which is a service that makes custom LEGO pieces to your specifications. I have no use for such at the moment, but the possibilities for game pieces are intriguing.

Hey Greg. Very excited by the prospect, so I dug a little deeper.

I just ran through the system at lego and while you can use it to get custom components (as long as they are made from the standard lego blocks), the hitch is when you go to order them.

First, each customer would have to order his or her own set, because they run with a max order of 1 per customer when ordering custom lego factory sets. What you get when you make the order is not just the pieces needed for your design, but one of the five standard sets of pieces, with directions for making your design from said pieces.

Prices for a factory set in the US is between $19.99 and $60-some-odd
