What happens if Simultaneous Reds are dropped, and both attackers fail in their rolls-do you drop Initiative again?

Yes. In fact, whatever the outcome of a red/red round (a single exchange, remember) you do initiative again.

Is that straight from the rules, Brian? If so, I must have missed it. In a Red-Red engagement, I always just let whoever succeeded better (assuming both succeeded) have the initiative. I don't think people always back off after a double-attack, assuming that the battle isn't over afterward.

Yup. I don't have the book in front of me right now to give you a page reference, but I confirmed it when I was coding the combat sim, that's how Jake said to do it.
