A question about Initiative

I have initiative and attack.
My opponent blocks and strikes. I miss him, and he hits me. Do I still have initiative?
What if I hit him, and he hits me. Who has initiative? Does who gets the better success or greater damage come into play?

Firstly.. you throw Red, and attack... he successfully blocks and therefore has initiative, he strikes..STOP!  
If he strikes and hits, he continues to have initiative until such time as he misses or you block or do something to steal initiative.

If you both throw red, you both attack.  I think some stat comes into play to determine who actually goes first.  Agility or something like that.  Then if someones left standing I think it goes off of whoever successfully hit, or failing that (if you both hit in order of Agility or whatever stat it was) the first person to strike.  I may be wrong about this second point though

Oops, I wasn't as clear as I could have been. Let me rephrase.

My opponent uses the "Simultaneous Block and Strike" manoeuvre. I miss him, and he hits me. Do I still have initiative?

If you miss you lose initiative, plain and simple.

If you both hit, then I'd reccomend that both of you do a full evade, but that's me. Technically, it's red-red until someone says "I'll defend." Context of reflexes, etc...real messy.

In the former case, if you miss, and he does not, then he has initiative.

In the latter case, where you both hit, I would go with whoever had the larger MoS on the hit. If they're the same, Then I'd go with whoever had the lesser Shock. If that's the same, Then go down to a contest of reflex, as per Double-Red engagements.

I hadn't thought of this approach. This way does make the "declare actions in order of lowest Reflex" rule make more sense - it doesn't matter so much when people are following the normal attack/defend routine.

So, both are attacking each exchange, but the slowest one declares first, and the fastest one has the choice: "do I defend, or do I strike - I'll hit first, so I get the chance to take him down before he hits me."

I just rule whoever hits last retains initiative, unless common sense dictates otherwise.  That being said, I like Jake's solution as well.
