Organization methods

So here I am, reformatting my hard drive. Over the past few years, about 30% of my drive space has mysteriously disappeared. I blame gremlins.

Anyways, while backing up my computer, I realized this was a perfect time to reorganize my files - but how? How do the rest of you, game smiths, organize your files? Do you have many documents or do you have just a few (perhaps 1 per chapter?) Somehow, I am just not happy with my current system and am looking for feedback.
